We have just emailed out our 6 month post evaluation feedback survey.    If you have not received yours, please check your spam and retrieve it, and if it’s not there, please tell us straight away!

Please look out for this email, and please please please complete the survey!  It is most important that we can fully evaluate the effects of the patient day  as it will help us get funding for future events.

If you don’t want to do the survey on-line, please get in touch with us and we can send a paper copy to you in the post.
